Report on Uvalde shooting 'Systemic Failures" #SystemicFailures #ActiveShooters #Police #uvalde HOUSTON — The first comprehensive assessment of the law enforcement response to the deadly school shooting in Uvalde, Texas, found that blame for the failure to swiftly confront the gunman rested not only with the school police chief, but also with the scores of state and federal officers who gathered at the deadly scene but did not act. The 77-page report, released Sunday by a special Texas House committee, represented a broad indictment of police inaction at Robb Elementary School, citing “systemic failures” that left the school inadequately secured and the police officers who responded mired in confusion and bad information. Nearly 400 officers responded to the school that day. Yet the decision to finally confront the gunman was made by a small group of officers, including specially trained Border Patrol agents and a deputy sheriff from a neighboring county, the report found, concluding that others at the scene could have taken charge and done so far earlier. The findings represented the most complete outside account of what took place during the 77 minutes between when the gunman began firing inside the classrooms and when the police finally stormed in and ended the May 24 massacre that left 19 students and two teachers dead. But the report found that a flawless police response would not have saved most of the victims, who suffered devastating injuries when they were shot with a high-powered AR-15-style rifle by a gunman who had been waiting for his 18th birthday to purchase the weapon legally. Some died on the way to the hospital, the report noted, adding in a final footnote that “it is plausible that some victims could have survived if they had not had to wait” for rescue. “If there’s only one thing that I can tell you is, there were multiple systemic failures,” State Representative Dustin Burrows, who spearheaded the investigation, said at a news conference on Sunday. “Several officers in the hallway or in that building knew or should have known there was dying in that classroom, and they should have done more, acted with urgency.” Mr. Burrows added that it would be up to the individual agencies to hold their officers accountable. The goal of the committee, he said, was to provide relatives of the victims and the public with information.

The fifth was reported on Jones Beach Island in neighboring Nassau County. None of the victims were seriously injured, and the sharks sighted were estimated to be four to five feet in length, officials said. Shark sightings have been increasing off U.S. coasts, which scientists attribute to successful conservation efforts that have restored populations closer to historical levels. Still, the recent spate of attacks was highly unusual — there were only 47 confirmed unprovoked attacks nationwide in 2021, according to the Florida Museum of Natural History’s International Shark Attack File. “This is not something that is precedented in any way in our history here in Suffolk County,” Suffolk County Executive Steven Bellone said at a news conference Thursday. “Prior to July 3, we had not had a recorded shark bite at [Smith Point Beach] since it had opened, during beach hours, back in 1959.” Bellone said the county is deploying drones and increased lifeguard patrols to monitor the waters, acknowledging that the uptick may represent the “new normal.” “The idea of more frequent contact with these kind of sharks may be what we will be coming to expect,” Bellone said at a separate news conference Wednesday, ahead of the fifth attack. The first of the recent attacks took place June 30, when a 57-year-old man swimming at Jones Beach suffered a laceration on his right foot that paramedics identified as a “possible shark bite,” according to the Nassau County Police Department. Three days later, Zach Gallo, 33, a lifeguard at Smith Point Beach, was playing the role of a victim in a rescue training exercise when he became one himself, WABC reported. He felt a roughly four-foot shark whip him with its tail and then bite his hand, according to the station. “I felt pressure in my hand, pulled it back and I just started hammering, punching and I connected with the shark three times, and then on the third time it spun away,” Gallo told WABC. “I guess my adrenaline, survival instincts kicked in.” Gallo returned to work Thursday, saying at the news conference with Bellone that he was grateful his injuries were minor and that his fellow lifeguards came to his aid. “If you do go in the ocean, make sure you are going into an area that’s protected by lifeguards,” Gallo said. On July 7, first-year lifeguard John Mullins, 17, was bitten on his foot while also playing the role of a victim during a training exercise near Ocean Beach, according to CBS New York. “The teeth were inside my skin and when I pulled my foot out, it kind of just felt like a scrape, like a rake going up my foot,” Mullins told the station. “We never expect to be attacked while we’re training, but they handled it well.” Mullins received five stitches and was out of work while his foot healed, CBS New York reported. On Wednesday morning, a surfer was bitten by a roughly four-foot tiger shark, leaving a four-inch gash, according to Bellone. The man was knocked off his board and saw the shark circling back toward him, but a wave carried him into shore, Bellone said. Roughly 11 hours later, police were called to Seaview Beach after a 49-year-old Arizona man standing in “waist deep water” was bitten from behind on his left wrist and buttocks, according to Suffolk County police. He walked out of the water and was taken by helicopter to a hospital with injuries that were not life-threatening, police said.

Statistics show that from 2000 to 2007, mass shooting incidents averaged 6.4 incidents annually, while from 2008 to 2013, they increased to 16.4. As a result of this increase, several studies have looked at the individual motivations and psychological “micro-foundations” that drive targeted violence–everything from video games to religious beliefs–in an effort to help law enforcement officials understand mass shooting, hate crimes, terrorism, and violence not only in the United States but also abroad. A 2015 study published in Behavioral Sciences and the Law, “The Concept of Identification in Threat Assessment,” examines the relationship between psychological “identification”–or the process by which an unstable person subsumes his or her own identity and models themselves after a violent aggressor–and acts of targeted violence. The researchers are based at the University of California-San Diego and the State University of New York Upstate Medical University as well as the San Diego Psychoanalytic Center, the firm Operational Consulting International and the Institute of Psychology and Threat Management in Germany. The authors drew on information from reviews and indirect assessment of evidence of criminal cases, as well as consultation with psychiatrists, psychologists and judicial officials who had direct access to primary investigative evidence on the cases. They also examined four detailed case studies on perpetrators of targeted violence: Antares Wong, Seung-Hui Cho, Joseph Paul Franklin and Anders Breivik. In analyzing the psychology of the four “active shooters,” the study authors drew on the work of psychiatrists Sigmund and Anna Freud and Erik Erikson on psychosocial development. The study’s findings include: The four perpetrators committed different acts of targeted violence, at different points in time, killing and injuring innocent people in the United States and abroad. Despite their differences, they all evidenced common expressions of “identification.” “In the context of threat assessment, identity becomes embedded in aggressive identifications, if not extremely violent images of the self in action, most often apparent in the fantasies of the young adult as a perpetrator of homicide against another.” Identification is characterized by one or more of five characteristics: pseudo-commando, warrior mentality, close association with weapons or other law-enforcement/military paraphernalia, identification with other attackers/assassins, and becoming an agent to advance a particular cause or belief system. Authors identified eight warning behaviors, including: Pathway warning behavior: research, planning, preparation, or implementation of an attack Fixation warning behavior: an increasingly pathological preoccupation with a person or a cause Identification warning behavior: a psychological desire to be a “pseudo-commando,” have a “warrior mentality,” closely associate with weapons or other military or law enforcement paraphernalia Novel aggression warning behavior: an act of violence that appears unrelated to any targeted violence pathway behavior which is committed for the first time Energy burst warning behavior: an increase in the frequency or variety of any noted activities related to the target Leakage warning behavior: the communication to a third party of intent to do harm to a target through an attack Last resort warning behavior: evidence of a violent “action/time imperative” Directly communicated threat warning behavior: the communication of a direct threat to the target or...
